Medical Information:	Please tell us if your pet suffers from any allergies/medical conditions and if they are on any medication.

* Medication:	I give consent for any medication described on this booking form, or any medication prescribed to my dog by a vet in my absence, to be administered to my dog by the person caring for my dog.

Vet Details:	Please include full contact details for your pet's vet. N.B. In the event of your pet becoming unwell, we will contact you or your designated person in the first instance. We will then make every effort to contact your registered veterinary surgeon. If your vet is unavailable we will contact a local vet who runs a 24 hour service and who has agreed to work with us to treat/look after your dog.

* Parasite Treatment:	In the event of parasites being found on my dog, I give my consent for my dog to be treated with an appropriate product authorised by the Veterinary Medicines Directorate and licensed for used in the UK. Treatment will be discussed with a veterinarian before administering. I understand I will be responsible for the veterinary costs and for transportation.
